New approaches for persistent pulmonary hypertension of newborn

The management of persistent pulmonary hypertension of newborn (PPHN) has entered a new era with the introduction of inhaled nitric oxide therapy and wider application of gentle ventilation, high-frequency oscillation, and surfactant replacement therapy. The use of invasive life-support measures such as extracorporeal oxygenation has decreased substantially in recent years. This article reviews the rapid progress being made in the understanding of the altered vascular biology in PPHN and the potential for more targeted therapies based on nitric oxide-cGMP signaling or on the prostaglandin pathway. This article focuses on the current evidence and future direction for research in this area.
