A left inferior vena cava(IVC) is an IVC anomaly with a prevalence of 0.2% - 0.5%. Typically, the left IVC joins the left renal vein, crosses anterior to the aorta, and unites with the right renal vein to form a normal right IVC. We report a case of an unusual left IVC with retrohepatic crossing over and intrahepatic venocaval shunts. In addition, situs ambiguous and polysplenia accompanied the anomaly. Though most IVC anomalies are discovered incidentally on imaging without relevant symptoms, knowledge about its variants is important to avoid misinterpretation on diagnostic imaging and to plan for surgical and interventional procedures.
