The disease management program (DMP) on "diabetes mellitus type 2" was introduced in 2002 with the aim of treating patients with diabetes according to evidence-based guideline standards to delay or prevent progression of the disease. In April 2004 more than 3 million patients in the national health insurance scheme were included in the program. The costs of medical documentation and coordination as well as insurance company administration were more than 500 million euros per year. Although the concepts and study protocols for a prospective randomized controlled trial (RCT) as a prerequisite for a valid evaluation were already planned for the introduction of a DMP, an RCT was never implemented. Therefore, so far no convincing comprehensive evaluation of the effects of the program exist. This article illuminates alternative evaluation approaches, such as the obligatory prescribed evaluation or propensity score matching for formation of control groups based on routine data from the national health insurance.
