The current outbreak of avian influenza A (H5N1) in poultry and,wild birds has spread to over 50 countries across Asia, Africa, Europe, and the Middle East. Between November 2003 and September 2007, there have been 328 confirmed human cases in 12 different countries with a mortality rate of 60% (200 fatalities). To date, the spread of the H5N1 virus from person to person has been rare, inefficient, and unsustained. The outbreak in poultry and wild birds is note likely to diminish in the near future, and, as a result, human infections with H5N1 resulting from direct contact with infected birds will continue to occur. Clinicians and laboratory professionals need to be aware of the possibility of avian influenza A (H5N1) infection, and they must understand and recognize the indications for laboratory testing. The rapid diagnosis of infected patients is critical for effective treatment and implementation of adequate infection control measures. Appropriate specimen collection, correct diagnostic testing, and effective communication with state and local public health authorities are all critical components of this diagnostic process.
