Although diabetes mellitus has a prevalence of up to 25% in the age group between 75 and 80years their health services situation remains unclear. In the LIVE-GERI-Study 32German ambulatory health care providing services (Amb n = 166 patients) and 40nursing homes in the region of Stuttgart (Hom n = 137 patients) participated. Mean duration of diabetes was 15 in the ambulatory, 9years in the nursing home setting, 75% of patients were female. Home care patients were more disabled and showed more geriatric syndromes. Diabetic complications were frequent in both groups. Blood sugar values were sufficient in both settings regarding medium fasting glucose (Amb 149mg/dl, Hom 145mg/dl). Basal supported oral insulin therapy (BOT) was performed in 5.4% (Amb) and 8.7% (Hom). Two thirds received a single insulin prescription, mainly premixed insulin. One third received a combination of short and long-acting insulin. 23.4% of patients in the Amb setting and 5.3% in Hom had regular visits by a diabetologist. In 62.3% of Amb and 83.6% of Hom the nursing staff had no goals for blood sugar. The study shows that in insulin-treated T2-diabetic patients in ambulatory or nursing home care very different strategies of insulin therapy are applied. Simple concepts as BOT are still underrepresented, diabetologists are rarely consulted and blood sugar elevations are not given enough attention.
