Maldigestion and malnutrition is a common finding in patients with long-standing chronic pancreatitis. Nutritional therapy is, therefore, crucial in these patients. In this Practice Point commentary, I discuss the findings and limitations of a prospective, randomized, controlled trial conducted by Singh and colleagues that compared the efficacy of dietary counseling for a balanced homemade diet with that of medium-chain-triglyceride-enriched dietary supplementation for the management of malnutrition in patients with chronic pancreatitis. The results of the trial are very interesting: over a period of 3 months, improvements in dietary intake, fecal fat loss, and pain score were observed for both groups with no significant difference between the groups for any parameter. Dietary counseling should become a cornerstone in the treatment of patients with malnutrition and chronic pancreatitis. In the future, nutritional intervention for patients with chronic pancreatitis and malnutrition should start with dietary counseling and be followed with medium-chain-triglyceride-enriched dietary supplementation if the former is not sufficient.
