Syncope, epileptic seizure, and psychogenic seizures are frequently encountered in medicine. A precise differential diagnosis is based on the history, and if possible, on the observation of an event. Pallor and visual blackout are in favour of syncope, closed eyes and manipulability during the event are in favour of a psychogenic seizure, and open eyes and the lateral tongue bite in favour of a true epileptic seizure. Myoclonic jerks occur in all three diseases, in epileptic seizures, they are typically rhythmic and brisk. Routine diagnostic procedures include, besides the history, a neurological and general medical physical examination, a blood analysis, an ECG, the Schellong test, and in our opinion also an EEG.
