Atypical Parkinsonian Syndromes With Very Poor Striatal Uptake on I-123-Ioflupane SPECT Imaging

We report a case series of unusual I-123-ioflupane SPECT imaging findings. Three patients with atypical parkinsonian symptoms underwent I-123-ioflupane imaging to assist in diagnosis. The patients had complete or near-complete absence of striatal activity. The imaging deficit severity did not consistently correlate with the clinical deficit severity. These cases suggest that markedly abnormal uptake patterns may be seen with atypical parkinsonian syndromes. Further research is warranted to evaluate differences in I-123-ioflupane uptake patterns among the various parkinsonian syndromes.
