Simultaneous Clear Lens Extraction and Silicon Oil Removal in Vitrectomized Myopic Eyes

M yopic eyes have a higher risk of retinal detachment, whether pseudophakic or phakic. 1 Because of the refractive index of silicon oil, patients with high myopia and retinal detachment experience improvement in their refraction after vitrectomy and silicone oil injection. After silicone oil removal, patients' refraction and visual acuity return to the original myopic status. This often led in our patients to disappointment and dissatisfaction particularly in patients who were not used to wear spectacles. Some of those patients came asking for intraocular surgery to correct their refractive error. A method combining cataractous lens extraction and vitrectomy for retinal detachment, simultaneously or consequently has been reported. 2-4 However, the experience of combined silicon oil removal and clear lens extraction in cases of myopic vitrectomized eyes is relatively limited. In this prospective study, we present the results of clear lens extraction (CLE) at the time of silicon oil removal in myopic patients who had prior pars plana vitrectomy for retinal detachment repair. We studied the feasibility of the combined procedure, accuracy of intraocular lens calculation, patient satisfaction, and risk of retinal re-detachment.
