摘要

Recently, endoscopic submucosal dissection (ESD) is carried out widely to treat early stage cancer in the digestive tract. Early cancer is accurately dissected as a whole by ESD regardless of its size, but it is a technically demanding procedure with a risk of complication. Therefore, lifting up the incised mucosa and attaining large field of endoscopic view are the keys to reducing the operation time and the risk of perforation. This paper presents experimental analysis and demonstration of a hood with hydraulically variable tip diameter for ESD. The hood is capable of changing its tip diameter by inflating the hydraulic actuator embedded inside the hood. The hood has a small tip diameter at its initial state, so that it may easily slip into the small incision made in the mucosa. By enlarging its tip diameter, the hood would lift up the lesion to incise, and the area to dissect could be confirmed through the endoscope. The tip diameter of the hood ranged from 9.8 to 17.8 mm when 0 to 250 kPa of pressure was applied. The hood generated 1005 mN of lifting force when 300 kPa of pressure was applied. The hood with hydraulically variable tip diameter was attached on the tip of an endoscope, and a demonstration of ESD was done inside a resected porcine stomach. The hood was able to generate force to lift up the lesion and expand the confirmable view of the submucosa.
