WHO /UNICEF recommends, based on evidence from large randomized controlled trials, use of WHO reduced osmolarity ORS with 75 mEq/l of sodium and 75 mmol/l of glucose (osmolarity 245mOsm/l) as the universal solution for all causes of diarrhea and at all ages. The recommendation also includes administration of 20 mg of elemental zinc as soon as diarrhea starts in children older than 6 months and 10 mg per day in infants less than 6 months. Zinc should be continued for a total period of 14 days. The Indian Academy of Pediatrics endorses these recommendations. Reduced osmolarity ORS has been introduced in the National Health Programme in 2002 and a policy decision has been taken for including zinc in the RCH/National Rural Health Mission by the Government of India in 2007. In the recent NFHS survey the percentage of children under age five with diarrhea in the two weeks preceding the survey was 9 percent. The big challenge today is to improve upon the current ORS and zinc use rates for treatment of diarrhea.
