The scientific soundness of guidelines is determined by the systematic approach of searching and reviewing the literature using the tools of evidence-based medicine. However, the acceptance of guidelines in practice is also determined by the involvement of experts representing all professions and patients addressed by the guideline and the application of formal consensus-development techniques. Additionally, guideline authors should adhere to international methodological standards and quality criteria for "good guidelines". These are summarized in the "german Instrument for Methodological Guideline Appraisal (DELBI), a checklist based on the AGREE-Instrument. In order to develop an evidence-based, high quality clinical practice guideline on atraumatic femoral head necrosis of adults a guideline-coordinating group of the Section of Osteology of the DGOOC (the German Association of Orthopaedics) carried out a systematic review of the literature. 2745 publications were identified by a systematic search covering the time between January 1991 and September 2009. All abstracts were screened using defined criteria of selection and the plausibility of the search strategy was controlled by additional hand-searches. 1026 papers remained, which will be assessed regarding their methodological quality and grade of evidence. Using these data, 30 questions will have to be answered which have been judged essential for the formulation of recommendations leading to adequate diagnosis and therapy of the disease by the coordinating group.
