Kawasaki disease (KD) is rare in neonates. We report two cases of neonatal KD, its course, therapeutic response and prognosis. The first case manifested with a fever and rash at the age of 12 days old followed by edema and peeling of extremities, other typical KD features and thrombocytopenia. He received IVIG, 400 mg/kg, three doses during the first week of illness prior to referral. His ECHO revealed dilated left coronary artery. The second case manifested with a fever at the age of 20 days along with rash and cervical lymphadenopathy, and later by chapped lips and peeling of extremities. He received IVIG 2 g/kg on 9th day of illness at our hospital. In this case coronary arteries were not involved. These reports highlight the thrombocytopenia and cardiac involvements in neonatal KD, and the need for early diagnosis and timely IVIG at 2 g/kg to avoid serious coronary complications.
