摘要

In retrospective study followed all TYPE 1 Diabetes mellitus (T1DM) patients with insulin pump (IP) therapy started in the period from the 1(st) March 2005 to the 1(st) September 2008.
We analyzed: age and sex structure of the observed patients; their therapy before IP use; total daily insulin dose during 6 months before and during 6 months after IP therapy introduced; incidence of severe hypoglycaemic episodes during 6 months before and during 6 months after IP therapy start.
The total number of observed patients was 39. There were 24 boys (61,5 %) and 15 girls (38,5 %) with the age range between 12,3 +/- 3,2 years. Most patients were from age group 8-14 years. Be e IP introduction most patient 61,5 % use therapy with insulin analogues. Mean daily insulin do e during 6 months before IP therapy was 0,96 +/- 0,12 IU/kg of body weight, and during 6 months long IP therapy was 0,61 +/- 0,07 IU/kg ( p =0,0044). Before IP therapy start severe hypoglycemia episodes occurred in 15 patients and after IP therapy in 2 patients. Frequency of severe hypoglycemic episodes decreased from 0,38 to 0,05 per patient. Incidence of severe hypoglycemic episodes was before LP start 0,82/patient/year and after IP start was 0, 10/patient/year ( p<0,001).
Therapy with IP in children with diabetes was very efficient in decreasing daily insulin dose and in significant decrease incidence of severe hypoglycemic episodes what would result in less variability of glycemia and in better metabolic control of the disease.
