Mini-Abstract There is an increasing prevalence of mechanical assist device use in patients with end-stage heart failure. With a growing number of intra- and postoperative cardiac thrombotic events in the setting of mechanical assist devices, recognition of this entity is becoming increasingly salient to the clinical management of this population. The case presented herein is notable for multiple reasons: (1) the thrombus demonstrated involves the entire left atrium, left ventricle, and proximal ascending aorta; (2) thrombosis did not occur in the setting of previously described triggers, including fresh frozen plasma, protamine, or intrinsic hypercoagulable state; and finally, (3) thrombosis occurred with therapeutic heparin levels.
