Fibular Allograft for Osteonecrosis Prevention in the Management of Femoral Neck Fractures: Clinical Outcome and Biomechanical Evaluation

Fibula allograft with cannulated screw is an effective regimen for preventing femoral head necrosis in the management of fracture neck fractures, which could solve the problems of mechanical stability, biological repair and reduce the intraosseous pressure of femoral head. In this study, the purpose was to evaluate biomechanical performance and clinical outcome of fibula allograft with cannulated screw procedure for femoral neck fracture. Six computational biomechanical models were created and used to simulate three fractures with two internal fixations. Stress distribution of cortical bone and total stress of femoral head were computed and used for biomechanical evaluation. Clinical outcome were used to confirm the biomechanical results. The results indicated that fibula allograft with cannulated screw procedure is a technically demanding procedure for osteonecrosis prevention in the management of femoral neck fracture.
