A new "chimney-like" surgical technique of reoperations of thrombosis of aorto-femoral and ilio-femoral bypass prostheses

The authors propose a new simplified surgical technique of thrombectomy of thrombosed aorto-femoral and ilio-femoral bypass. In case of a graft, totally enveloped in the periprosthetic connective tissue, the authors unroof only the front side of the thrombosed vascular graft. An oval opening is cut on the anterior wall of the prosthesis and a small prosthetic segment (called chimney) is sutured in. In order for haemorrhage to be under control, the chimney is placed on a tourniquet and a thrombectomy of the proximal segment of the prosthesis and anastomosis is performed through it. A thrombectomy of the distal fragment and anastomosis follows. If the blood flow is adequately normalized and the passage is considered satisfactory the chimney is clamped and removed. If the pathologic process has a distal progress, the chimney is used as a proximal segment of a new aorto-femoro-popliteal bypass. This method gave excellent results in 24 cases.
