Multidrug resistance is causing a therapeutic crisis in human medicine particularly in hospital environments but, as yet with few exceptions, there is not a therapeutic crisis in veterinary medicine. Concern about the possible transfer of resistant commensal and zoonotic bacteria and particularly resistance genes (through plasmid-mediated lateral transfer and/or clonal expansion), is driving the current scientific and political debate. While the major cause of resistance in human medicine is the use of antibiotics in humans, it is important that veterinarians also husband available antimicrobial resources through responsible use and antimicrobial stewardship programmes. The aim for any future interventions in the veterinary use of antimicrobials should be firstly to improve human health, and secondly to improve animal health. Progress should be measured against these aims and not surrogates such as broad 'reduction targets' which may not be closely linked to the primary objective. In the design of any interventions the possibility of otherwise unforeseen consequences should be taken into account, otherwise the eventual outcome could be negative impacts on animal health and welfare and even on public health.
