Although rare, hairy cell leukemia (HCL) can be positive for CD10; thus, differential diagnosis of CD10(+) small mature B-cell lymphoma should expand from follicular lymphoma and Burkitt lymphoma to include HCL. Interpretation of immunophenotype revealed by flow cytometry always has to be correlated with cytomorphology to avoid misdiagnosis. Although rare, HCL can involve extramedullary tissues especially lymph nodes; thus, when a fine-needle aspiration is performed on a lymph node, be aware of nodal involvement by HCL. A multimodality approach including clinical presentation, imaging, flow cytometry, and cytomorphologic assessment is needed for working up B-cell lymphoma.
