Cerebellar encephalopathy and acute psychosis associated with infusion of DMSO preserved stem cells

A 22 year-old male patient with stage II B lymphoblastic lymphoma underwent high dose chemotherapy (oral busulphan and cyclophosphamide) and autologous peripheral stem cell transplantation. On day +1 cerebellar signs (vertigo, ataxia, nystagmus, dysmetria, dysdiadochokinesia, dysarthria, myoclonus and nervus abducens paralysis) developed along with psychiatric findings such as anxiety and sleep paralysis that over days progressed to frank psychosis with agitation, visual and auditory hallucinations, persecutory delusions and self-harming behavior. DMSO (Di-methylsulfoxide) infusion was considered to be associated with this fatal complication.
