Analysis of the Changes in Photoplethysmogram Induced by Exercise Stress

The level of physical stress was usually monitored by directly comparing the subject's heart rate with an appropriate "target heart rate" during exercise. It was hypothesized in this paper that waveform feature of Photoplethysmogram (PPG) might be useful for noninvasively detecting stress to exercise of cardiovascular system. Exercise stress was imposed on ten healthy subjects by bicycle ergometry test. Approximately half an hour Electrocardiogram (ECG) of lead V5 and PPG of left finger was obtained simultaneously from each subject, which was consisted of baseline period (5 minutes' relaxation before exercise), workload period (20 minutes' exercise on bicycle ergometry), and recovery period (5 Minutes' relaxation after exercise). Discrimination abilities of 21 measurements from PPG and ECG were tested for distinguishing the states between exercise and relaxation in terms of waveform amplitude, time span and slope. Morphology changes were observed in the finger PPG in all subjects during exercise period in comparison with that during baseline period. And PPG waveform restored partially during recovery period. As a result, ten indices with significant differences (analysis of covariance, p < 0.05) between exercise and relaxation were selected as markers of stress level from the 21 parameters, which contained eight time span indices, one slope ratio and one area ratio. Furthermore, 16 indices out of the 21 indices were divided into two groups, while strong correlated relationship existed in inner-group, but poor relationship between the groups. In conclusion, not only PPG rhythm but also PPG waveform parameters had the discrimination ability to distinguish exercise state from relaxation state.
