Population Pharmacokinetics and Pharmacodynamics Analysis to Optimize Dosing Regimens of Cefepime

The pharmacokinetic/pharmacodynamic (PK/PD) index that best predicts the activity of antibiotics is the time during which the plasma concentration remains above the minimum inhibitory concentration (T->MIC) of the microorganism. The aims of this study were to: (i) demonstrate the population PK, and (ii) assess the efficacy of recommended dosing in achieving the probability of target attainment (PTA) and the cumulative fraction of response (CFR) of cefepime. The study was conducted in 18 healthy volunteers. The volume of distribution and total clearance of cefepime were 14.25 +/- 1.35 L and 5.49 +/- 1.20 L/h, respectively. The PTAs (>= 90%) for a target of 60% T->MIC with a MIC of 4 mg/L were observed and the predicted CFRs for achieving a target of 60% T->MIC for E. coli and S. aureus were >= 90%. In conclusion, all dosing regimens could provide good coverage for pathogens with a MIC of 4 mg/L and had a high probability of achieving optimal impact against E. coli and S. aureus.
