Atopic dermatitis (AD) is often associated with sensitization to aeroallergens, but the role of sensitization in the development of eczema is still controversial. However, in a subgroup of patients with AD, eczematous skin lesions can be induced by epicutaneous testing with aeroallergens, a procedure named Atopy Patch Test (APT). Exposure to aeroallergens can exacerbate or maintain the disease, and APTs are probably useful to identify this triggering factor. APTs show a higher specificity than skin prick-tests. IgE-sensitization is not a necessary prerequisite for the development of AD. Avoidance of house-dust-mite does not show any preventive effect on the onset or progression of AD. The studies are generally negative in primary prevention and inconsistent in secondary prevention.
