We report the case of a 58-year-old SNCF employee who developed occupational asthma following exposure to fire extinguisher powder. Spirometry revealed no evidence of airway obstruction. The initial methacholine bronchial provocation test was also negative. A provocation test with exposure to fire extinguisher powder carried out in an environmental challenge chamber was definitely negative, but a second methacholine provocation test carried out 24 hours after that test clearly demonstrated the presence of non-specific bronchial hyperreactivity. Notification to the Social Security of an occupational disease was not possible because the responsible agent was not mentioned in the limited list of Table 66. A change of his occupational position by the SNCF reduced but did not completely eliminate his respiratory symptoms.
