Acute myocardial infarction is the most commonly found cause of death during medicolegal autopsies ordered to investigate cases of sudden unexpected death. In many cases a medicolegal examination is ordered to explore whether a myocardial infarction may have been overlooked. To adequately answer this question the medicolegal examiner has to be aware of the current standards in the diagnostics and treatment of acute myocardial infarction. In view of the immense significance of coronary heart disease as a public health challenge with only slow improvement in the morbidity and mortality rates, the last few years have seen the development of validated, high quality healthcare guidelines that are based on extensive data and characterized by a high measure of interdisciplinary consensus. These guidelines can serve the medicolegal expert as a compass in the evaluation. In finding an answer to the far-reaching forensic question whether the life of a patient could have been saved or at least have been prolonged if a myocardial infarction had been diagnosed in time for successful treatment, criteria from evidence-based medicine should be considered along with the specifics of the particular case.
