Adverse effects after metal-on-metal bearings in total hip arthroplasty

Adverse effects after metal-on-metal (MoM) total hip arthroplasty (THA) are a matter of concern. The aim of this study was to systematically analyze the literature on these events and perform a structured review on the topic. We conducted a systematic, online search using PubMed and MEDLINE using the search terms adverse [All Fields] AND reactions [All Fields] AND debris [All Fields] AND %26quot;hip%26quot; [MeSH Terms] OR %26quot;hip%26quot; [All Fields]. Data on the endpoints clinical and radiological outcome, failure, and complications were extracted and summarized. The systematic search revealed 48 manuscripts in total. After exclusion of duplicates and irrelevant publications, and inclusion of further studies identified by reviewing of bibliographies of all included studies, 67 studies remained for final analysis and inclusion in this review. All studies were published in English from 1992 to 2012. The analysis showed a significant amount of adverse reactions after MoM TEA or RHA and various cases having undergone surgical revision for aseptic loosening or adverse reactions to metal debris. Adverse reactions after metal-on-metal total hip arthroplasty remain a matter of concern. The authors believe that facing a huge variety of different bearing surfaces, only those without significant side effects should be chosen.
