Displacement of Dental Implants into the Maxillary Sinus: An Overview and Case Series

Objectives: An increasing incidence of displaced dental implants in the maxillary sinus is observed as a complication during insertion and healing in the last years. The aim of this study is to categorize the types of the implant displacement in the maxillary sinus and discuss the different therapeutic approaches.
Case Description: We analyzed implant displacement into the maxillary sinus in four patients. Due to the anatomical location the displaced implants were divided in two groups: an intraantrale and a submembraneous group. The displacement of the implant occurred in three patients during the crestal placement of implants, in one case in the postoperative period (1st week after surgery). In two cases, the displacement was intraantral in the lumen of the maxillary sinus, in two other cases under the mucosa of the sinus maxillaris.
Summary: Especially in vertically resorbed bone ridges the risk of implant displacement in the maxillary sinus is increased. Depending on the type of displacement the implant can even remove without the aid of an endoscope.
