Anaesthesia in epileptic patients

Nowadays, most of the anaesthetic drugs commonly employed, show a neuroprorective effect thanks to the central nervous system depression that they induce. Considering the patients' health status, benzodiacepines, alpha(2)-agonists or even phenotiazine derivates (traditionally contraindicated) could be chosen. The use of analgesic drugs, both opioids and nsaid's mixed with tranquilizers, decrease the probability of seizures, resulting in an extremely recommended combination. The status epilepticus refractarius has been treated with barbiturates and propofol. These drugs could be used for the anaesthetic induction, being the propofol even adequate for the maintenance. It has been recently reported the utility of ketamine in prolonged situations of refractory epilepsy. However, its epileptic potential is well known, not being recommended in these patients. Among all the inhalant anaesthetics; normally employed for the anaesthetic maintenance, enflurane has been the only one showing its excitatory properties and its seizure capacity. On the contrary, isoflurane has been considered the safest inhalant anaesthetic in the epileptic patient.
