A Clinical Study of the Change of Stiffness of Cystic Thyroid Nodules After Percutaneous Ethanol Injection with 2D-ShearWave (TM) Elastography

The purpose of the study was to monitor the stiffness of cystic thyroid nodules (CTNs) after percutaneous ethanol injection (PEI) with two dimensional-ShearWave (TM) Elastography (2D-SWE; SuperSonic Imagine, Aix-en-Provence, France) and investigate the characteristics of stiffness change of CTNs after PEI. Thirty consecutive patients with 30 CTNs between August 2014 and June 2015 were selected in the study. After being confirmed as benign nodules by fine needle aspiration cytology (FNAC), PEI was performed in all patients. Conventional ultrasonography (two-dimensional ultrasonography and color Doppler flow imaging) and 2D-SWE examination were carried out at 24 hours and 30 days after PEI. The transverse and longitudinal sections of the solid areas of CTNs were selected for shear wave stiffness (SWS) measurement and then the average was calculated. The Wilcoxon's matched pair signed rank test was used to compare the indexes of symptomatic, cosmetic and ultrasonographic evaluation of the nodules at 24 hours and 30 days after PEI. SWS of the solid areas of CTNs (SWSs) measured at 24 hours after PEI was (18.03 +/- 5.44) kPa, and 30 days was (21.48 +/- 6.05) kPa. SWSs measured at 30 days was significantly higher than 24 hours (p = 0.003). However, there were no significant differences of the results of symptomatic, cosmetic and conventional ultrasonographic evaluation (p>0.05). The study suggests that after PEI treatment, the stiffness of CTNs changes significantly within 30 days. However, there are no significant changes of the indexes of symptomatic, cosmetic and conventional ultrasonographic evaluation. That is, the change of nodule stiffness is more sensitive than other indexes.
