Coronary chronic total occlusions (CTO) are defined by TIMI 0 flow for at least three months. Percutaneous coronary intervention (PCI) of these lesions is completely different from the systematic late opening of a myocardial infarction coronary vessel. Indication for CTO treatment is mainly based on symptoms/ischemia and myocardial viability in the territory of the occluded vessel. To date, the benefit of the treatment has not been demonstrated in a randomised study vs. medical treatment by intention to treat analysis, but comparisons between success and failure of vessel recanalization show a benefit in terms of symptoms and ischemia, a reduction in mortality, MI and coronary surgery, and an improvement in ventricular function when it is reduced. PCI of these lesions is extremely complex and requires various specific devices, techniques and strategies. As operator experience is a major predictor of success, it is recommended that these procedures should be performed in high volume centers by dedicated operators.
