During the initial examination of anemic patients, pediatricians need to differentiate between chronic (e. g. iron-deficiency anemia) and acutely severe, life-threatening anemia, which necessitates rapid clarification (e. g. hemolytic anemia). We present the most important algorithms for a target-orientated diagnosis and appropriate therapy with respect to current guidelines that significantly facilitate diagnostics in pediatric practice. It is currently recommended to be relatively restrictive when assessing the indications for blood transfusion, which is normally in cases of acute hemolysis with hemoglobin concentrations below 6.0 g/dl (3.7 mmol/l). Due to migration, sickle cell disease, glucose-6-phosphate dehydrogenase deficiency and thalassemia are also becoming more important in Germany. Pediatricians need to be familiar with these aEuronew" anemias that are not so rare in migrant populations in order to promptly detect severe complications.
